Anthem Blue Cross Blue Shield is placing policy holders on notice —in Kentucky, Missouri, and Georgia- that they could be responsible covering the cost for certain ER visits. Anthem has developed a secret list of diagnoses they will not cover, even if the patient thought it was a medical emergency.
